We encourage each member and regular attendee to get a name tag and wear it at church worship, meetings, and other parish functions.  If you need a new name tag you may submit a Name Tag Request.  A paper form is also available in the Narthex.  Your new tag should be ready in a two or three weeks.  Keep an eye on the tag-board in the Narthex.  Tags are kept in semi-alphabetic order by last name.

Required fields are marked by “*”.  If you specify nothing for “How you want do you want your name to appear” then your First (or Goes-By) and Last Names will be used.  If the tag is for a child,  This is where the child's name goes.
