We encourage every member to get a name tag and wear it at church worship, meetings, and other parish functions.  If you need a new name tag you may fill out the form and submit it.  A paper request form is also available in the Narthex.  Your new tag sould be ready in a two or three weeks.  Your patience and understanding is greatly appreciated.  Keep an eye on the tag-board in the Narthex for your new tag.  Tags are kept in semi-alphabetic order by last name.

The only required information on the Nametag Request are your First Name, Last Name and Email Address.  If you do not specify anything in the space labeled, “How you want do you want your name to appear on the tag?”, then your First Name, Middle Name (if supplied) and Last Name will be used.  For instance, this is where you would put your nickname and last name.  If the tag is for a child, this is where the child's name would go.
